About Us

Deb Soltis, founder of Soltis Senior Care Connections, had spent nearly two decades working with seniors and their families in a variety of capacities in both retirement communities and home health care. It became apparent that seniors and their families needed objective individualized support which offered them referrals and options based on their personal wants and needs. She had frequently witnessed the stress and concern caregivers had for their loved ones, especially those living at a distance. She also realized the need for those who have no family to have an advocate who could assist them with the challenges of aging.

Deb Soltis and her professional associates have a passion for the elderly and share a commitment to see each of our clients living their ultimate life capacity.

For our family, this service has meant the freedom to pursue career opportunities while knowing our father is individually supervised and encouraged apart from the assisted living staff. We visit often, but knowing that we have a local contact for emergency assistance or simply to visit during the day is reassuring and alleviates some of our concern for my father’s health and well being.

DAUGHTER OF A SSCC CLIENT

It was a great relief to engage Deb Soltis to coordinate my sister’s care in Pennsylvania while I resided in Arizona. Deb not only took a personal interest in my sister, but saw that she kept her medical appointments; interviewed possible care givers with my sister; acted as her advocate when needed; and continually provided me with updates on how things were going. I enthusiastically recommend Deb without hesitation to anyone needing professional and personalized help with anyone needing care.

SOLTIS SENIOR CARE CONNECTIONS PROCESS

Step 1: Assessment

Service begins with a comprehensive assessment of an individual's current circumstances and existing support system. The assessment is conducted through interviews with the client and other relevant contacts, observation (usually in the form of a visit to the client's residence) and review of additional information provided by the client or family member.

Step 2: Presentation of Care Plan

Following the assessment, a summary overview and a set of recommendations or a care plan is prepared and presented to the individual and/or caregiver. The care plan is prioritized to ensure that the most pressing needs are addressed first.

Step 3: Providing Service or Referral

There are 2 ways to proceed with care plan recommendations:

  • For families who wish to secure services through outside providers or provide care themselves, Soltis Senior Care Connections will make available a list of suggested organizations and providers to be contacted by the client or family.
  • Soltis Senior Care Connections can provide selected services while referring the family or making direct arrangements for service through third-party providers including the oversight and monitoring of such services.

Most often, it is the combination of services that creates the most effective long-term solutions and positive outcomes.
